Otosomal Dominant Polycystic Renal Disease And Intracranial Aneurysms: Current Opinions On Screening Strategies

aNöroloji AD, Dr. Edip Aktin İnme Ünitesi, Istanbul Üniversitesi İstanbul Tıp Fakültesi, İSTANBUL Due to its prognostic implications, intracranial aneurysms are the most important neurological pathology in autosomal dominant polycystic renal disease (ODPRD). When compared to the general population subarachnoid hemorrhages occur at a younger age and a familly history is present in nearly half of the patients. In angiographic screening studies, the incidence of asymptomatic, unruptured intracranial aneurysms (AUIA) is 5 to 10%, being clearly higher than the general population. But like the general population, the majority of these AUIA's are smaller than 6 mm, suggesting a low risk of rupture. Despite the high rate of AUIA incidence, the benefit of a screening strategy is not yet demonstrated. The decision should be individualized but the yield of the sceening is clearly increased in the presence of a familly history. Magnetic resonnance angiography has a high sensitivity and specificity and seem to be the best screening tool due to the absence of the need of contrast injection.
