Chronic Kidney Disease: Natural Course And Mechanisms For Retarding Its Progression

aİç Hastalıkları AD, Nefroloji Ünitesi, Hacettepe Üniversitesi Tıp Fakültesi, ANKARA The incidence and prevalence of chronic kidney disease (CKD) is increasing both in the world and in Turkey. Whatever the underlying disease, the major problem in CKD is the relentless progression of kidney failure to end-stage renal disease (ESRD). It is extremely important to know the natural course of CKD and to stop or slow down the progression of CKD to ESRD. The progression rate of CKD varies in each patient and each underlying disease. There are both unmodifiable and modifiable factors mediating the progression rate of CKD. Among these factors, blood pressure and proteinuria appears to be most important regarding therapeutic interventions. During the last 20 years, great progress was made for renoprotection by applying experimental data of renin-angiotensin system inhibitors to the clinical practice. However, to stop the progress of kidney disease completely, a clinical action plan is required that includes a multifactorial approach. The most important fact for stopping the progression of renal disease is absolute prevention of renal disease, as “no treatment is better than being disease-free”.
