The Role Of The Kidneys In Blood Pressure Homeostasis-i: The Raas

Full appreciation of the contributions made by the kidneys to the development of hypertension mandates appreciation of their contributions to the preservation of blood pressure in health. This review will focus on the leading effector of blood pressure homeostasis, the renin-angiotensin-aldosterone system (RAAS). The main purpose of this review is to summarize the effects of the RAAS in the maintenance and regulation of blood pressure in the healthy state and to thereby clarify the possible contributions made by the kidneys via the RAAS during the development of hypertension and target organ damage. The fact that while short term and modest activation of the RAAS is highly beneficial, its long term and excessive activation is dangerous will be emphasized. Since the RAAS is a physiological mechanism of vital value, another purpose of this review is to draw attention to the potential risks of drugs that interfere with or attenuate its activity, to provide stimulus for increased vigilance during their use.Keywords: Kidney, renin, angiotensin, aldosteron, hypertension, remodellingTurkiye Klinikleri J Int Med Sci 2005, 1(38):1-8
