Renal Parenchymal Hypertension In Adults
Dr. Mehmet Rıza ALTIPARMAK,a Dr. Ekrem EREKa
aİç Hastalıkları AD, Nefroloji BD, İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi, İSTANBUL Renal parenchymal hypertension is the most common cause of secondary hypertension in adults (2-5%). Hypertension is more frequent among patients with chronic glomerulonephritis, diabetic nephropathy, chronic interstistial nephritis and autosomal dominant polycystic kidney disease. Renal parenchymal hypertension is associated with a more rapid progression of renal damage, regardless of the underlying renal disease. Systemic hypertension in renal diseases is the result of a derangement in the handling of sodium and water leading to volume expansion and/or an inappropriate activation of the renin - angiotensin - aldoterone system. Agressive blood pressure reduction (<130/80 mmHg) is needed to slow down progression of renal disease. Antihypertensive regimens should include an ACE inhibitor in order to provide maximum renal benefits in these patients. Keywords: Renal parenchymal disease, secondary hypertensionTurkiye Klinikleri J Int Med Sci 2005, 1(38):50-52
