Pharmacokinetics Of Antihypertensives And Principles Of Antihypertensive Medication Use In Peritoneal Dialysis Patients

aİç Hastalıkları AD, Nefroloji BD, Akdeniz Üniversitesi Tıp Fakültesi, ANTALYA The general principles of hypertension treatment in peritoneal dialysis patients are not much different than those pertaining to hemodialysis patients. In most instances, hypertension in peritoneal dialysis patients is closely related to hypervolemia. Generally, antihypertensive treatment in peritoneal dialysis patients should first target the elimination of hypervolemia and patients should be strongly advised about the importance of non-drug measures such as salt restriction. As a modality-related advantage related to its being a continuous form of dialysis, peritoneal dialysis stabilizes blood pressure without causing the rapid, wave-like fluctuations that are common in hemodialysis. On the other hand, eventual loss of ultrafiltration capacity may be an important contributor in the hypertension problem of peritoneal dialysis patients, especially those on long-term treatment. Since adjusting drug doses according to the residual renal clearance of patients and the amount of drug removal via dialysis is the mandatory rule in patients with end stage renal failure and/or on dialysis, the table included in this review should be consulted with respect to drug removal through dialysis and drug interactions should be considered before establishing a certain antihypertensive therapy regimen for PD patients.Keywords: Peritoneal dialysis, hypertension,antihypertensives, durug kineticsTurkiye Klinikleri J Int Med Sci 2005, 1(38):120-122
