Acute Interstitial Nephritis

aNefroloji Kliniği, Trabzon Numune Eğitim ve Araştırma Hastanesi, TRABZON Tubulointerstitial diseases constitute a diverse group of acute and chronic hereditary and acquired disorders involving renal tubules and supporting structures. Drugs are a leading cause of this type of acute renal failure, identifiable by acute oliguria and sometimes fever, rash, and arthralgias. In addition to azotemia, tubuler dysfunction may be present. Common causes are methicillin and other penicillins, sufonamides, diuretics, rifampin, cimetidine, cephalosporin, and allopurinol; non-steroid anti-inflamatuar drugs (NSAIDs) cause acute interstitial nephritis (AIN) with nephrotic syndrome that may lack allergic manifestations. On renal biopsy, interstitial edeme with white blood cells (WBC) infiltration is present. Renal failure commonly responds to withdrawal of offending drug, and most of them have good recovery.
